FIRST APPOINTMENT: WHAT TO BRING

APPOINTMENT FORMS

First Appointment

Our office staff will brief you on how to prepare for your first visit and what to expect. We value your time with us and try our best to make your appointment a smooth, non-problematic experience.

New patients seeing us for the first time are encouraged to complete our new patient forms. You can get started on your forms by clicking on the link below:

What to Bring:

  • A valid ID
  • Insurance card (if applicable)
  • Last set of x-rays, if taken within the last 6 months
  • A parent, for children under 18 years of age
  • Anything to improve your comfort such as magazines, blankets, earphones, etc.

How long will the appointment take?

If you are visiting for an exam, your visit can last an average of anywhere between 60-80 minutes. For a consultation, the visit will take less time, which can approximate between 30-60 minutes.

What to expect.

Your new dentist will perform a complete oral exam and discuss treatments that may work for you. For additional information on the exam process, see our services page. We welcome any questions you may have regarding your expectations and goals.

A professional cleaning may be performed during the visit. The cleaning will help prevent future oral health problems such as cavities and periodontal disease.

Your treatment coordinator will assist with additional concerns about financing and scheduling of the next appointment.
